Forms

While OHIP covers medically necessary services, certain administrative tasks—such as sick notes, insurance forms, and other documents—are considered uninsured services and cannot be billed to OHIP. Due to high demand, please allow up to 45 days for processing. In some cases, pre-payment may be required.

Once your form is completed, our team will contact you to arrange payment and pick-up. Forms will only be released after payment has been received.

We kindly ask that you refrain from calling multiple times for updates and that all interactions with our staff remain courteous and respectful. Processing times are determined by physician availability, and delays do not indicate negligence. As noted, forms are not considered urgent matters, and physicians have up to 45 days to complete them.
